Program Curriculum

The 成人VR视频 General Internal Medicine Residency Program, like all Royal听College-accredited GIM programs, fully transitioned to the Royal College Competence by Design听(CBD) framework in July 2019. The minimum amount of training time is 2 years, with funding for extension of training being assured in cases where the 成人VR视频 GIM Program Competence Committee and Faculty Program Promotions Committee/Competence Committee deems necessary.

The 成人VR视频 GIM Program includes all of the training experiences required by the Royal College in order to provide the basis for achieving competence in all of the GIM Entrustable Professional Activities (EPAs).听 These are available on the Royal College website.

Practically, residents must complete required training experiences nested within stages of training. As befits a generalist specialty, there are extensive听elective opportunities and these provide breadth and depth to subspecialty consultant expertise and/or procedural and/or academic听training, tailored by the resident themselves to their听chosen career path. Times within specific CBD stages of training may be adjusted by the Program Competence Committee depending on the resident's academic portfolio.

Four core teaching hospitals: 成人VR视频 Health Centre (Montreal General Hospital and Royal Victoria Hospital-Glen site), the Sir听Mortimer B. Davis Jewish General Hospital and St. Mary's Hospital Centre provide training, as do a large number of collaborating hospital sites which offer rich training opportunities in community ICU and non-metropolitan.non-tertiary "community" GIM.

Rotation specific details below:

Jr Attending Team Leader Rotation

The resident will spend 8 weeks听as a Jr Attending on one of the Clinical Teaching Units (CTU) (at听JGH, MUHC听or St. Mary芒鈧劉s) and/or on one of the ED GIMCS services (at JGH or RVH)听during the two-year training period. The rotation can be done as a combination of 2-week blocks (i.e. 4 times 2 weeks or 2 times 4 weeks, or combinations thereof).

The faculty attending must听be a member of the Division of Internal Medicine (exceptions must be reviewed and authorized by the Program Director).

The second 4 weeks must occur in a different site than the first rotation so as to ensure a broader experience in terms of CTU settings and academic staff. The resident will receive the CTU Attending schedule of the four hospitals as soon听as they are available in the spring and will indicate their top four choices for co-attending site and period to the coordinator of the General Internal Medicine Program (Audrey Weizmann, 514-340-8222, ext. 23942, aweizman [at] jgh.mcgill.ca).听Every effort will be made to honor the resident芒鈧劉s preferred sites and periods for Jr attending. Once the schedules are finalized, the resident will be notified of where they will be Jr attending and during which periods, as will the respective CTU attendings.

It is the resident芒鈧劉s responsibility to contact the attending with whom he or she will be working with at least 2-3 months prior to the start of the rotation in order to review their role on the CTU (please refer to goals and objectives for Jr Attending Team Leader rotation). The resident should also be familiar with the Department of Medicine Guidelines for Attending Physicians at their respective hospitals. Please see Appendix 芒鈧揃芒鈧 describing the attending physician and trainee expectations for the Jr Attending Team Leader rotation.

It is the resident's responsibility to advise the attending 1 week before the start of the rotation of which home听call days they will take, including weekends/stat days at the 3 sites (JGH, MGH, RVH).听 It is also the resident's responsibility to notify Locating/Call Centre on the weekend/stat days in question to say that they are first call for that day.

The resident is expected to continue with his/her weekly half-day ambulatory clinic (JGH and MUHC), and attend all academic activities including the Clinical Scholars Program and the academic/division rounds presented at the sites of their听rotation.听

Jr Attending Rotation - expectations :

Critical Care (ICU/CCU)

The resident is expected to complete 8 weeks of critical care medicine during their 2 years of GIM training and can begin to do so in the Core of Discipline stage of training. These can either be in ICU or CCU. The ICU rotation has to be booked through the Program office. The resident will be asked to submit their request by mid-May indicating a choice of periods and sites. As for the CCU rotation, it is the resident芒鈧劉s responsibility to arrange this rotation and to notify the Program office once it has been booked. This should be done early in the academic year. The resident will generally function as Critical Care Fellow during that period, supervising or co-supervising (i.e. if there is a cardiac or critical care fellow also assigned that month) the team of residents.

CCU: At the JGH and RVH, the GIM residents may be expected to participate in junior call. At the MGH, GIM residents will be scheduled for call duties from home, with the understanding that the resident is expected to be in-house when patient care requires it and is expected to return to the hospital for unstable admissions and at any time when the more junior trainees need support. The resident who has spent most of the night in hospital will be granted leave on the following day, in accordance with the resident芒鈧劉s collective agreement. It is highly recommended that the resident not take any leaves while on the CCU rotation. The CMR at each site is responsible for the call schedule and all call requests go to them (CMR.JGH [at] gmail.com, CMR.MGH [at] gmail.com or CMR.RVH [at] gmail.com).

ICU: During the 1st month in the ICU, the GIM resident will be scheduled for night duty in-hospital. This applies to all three sites (JGH, MGH & RVH). The resident will be scheduled for night duty from home in subsequent rotation months. Please see Appendix 芒鈧揂芒鈧, the new ICU rotation scheduling policy document prepared by Dr. Hornstein. General Internal Medicine Consult Service

The resident can complete 4 weeks听in general internal medicine consultation service (GIMCS) during the Transition to听Discipline (TTD) stage of training;听8-12 weeks in the听Foundations of Discipline (FoD); and 8 weeks in the Core of Discipline (CoD) stage of training with a minimum听of 4 weeks in a community听centre with few non-GIM medical subspecialists. This听Required Training听Experience (RTE)听can be done at any of the primary training sites, or other sites while in CoD stage听upon approval of the Program Director, with no more than 4 weeks at any one site. The resident will be asked a choice of periods and every attempt will be made to honor these choices when the Program Office books the rotations.听 Rotations can be done as combinations of 2-week blocks (i.e. 6 times 2 weeks, 3 times 4 weeks, or combinations thereof).

Home call is expected during rotations at JGH, MGH, RVH and Lakeshore General Hospital.听 The scheduling varies at the three sites. The regulations for the maximum number of home calls, including the maximum number of weekends, is clearly outlined in the FMRQ collective agreement.听 This maximum should not be surpassed.

At the JGH, the GIMCS rotation incorporates 2 week-end calls. The resident can request which week-end they would like off.听 The week-end call will consist of call on both Saturday and Sunday.听 It is expected that the resident come in the morning, round on the service, see new consults and review with staff on call.听 The coordinator at the JGH will ask for call requests 2 weeks prior to the start of the rotation.听 There is no week-day home call at the JGH.听

At the MGH, the resident is required to do 1 week of evening call (Monday to Friday).听 The resident is also required to do 1 week-end call which begins on Friday evening until Sunday night.听 The resident has the choice of which week and week-end they would like to do their call duties and can let the coordinator at the MGH know.

At the RVH, the GIMCS rotation incorporates 2 week-end calls. The resident can request which week-ends they will work.听 The week-end call will consist of call on both Saturday and Sunday.听 It is expected that the resident come in the morning, round on the service, see new consults and review with staff on call. The week-ends on call should be selected 1 month prior to the start of the rotation and emailed to the director of the service at emily.mcdonald [at] mcgill.ca. During a 4 week block the resident is encouraged to select 4 evenings where they will take home call. These do not need to be selected in advance of the rotation.

At the Lakeshore General Hospital, the resident is required to do two weeks internal medicine consultation service in the ER as well as two weeks of consultation service on the wards. The resident is required to do one weekend call; no evening call.

Pre-Operative Medicine

A one month rotation in outpatient preoperative medicine clinics is mandatory.

There is no inpatient component and the rotation will consist of daily 脗陆 day clinics where the resident will see patients scheduled for elective surgeries.听

The clinics are integrated across the following sites: JGH, Lachine, MGH, MNH, RVH and St. Mary芒鈧劉s. The template below indicates the time and site of the various preoperative outpatient clinics, as well as the contact persons at each site. You are required to attend the various teaching sessions organized at each site.

A reading list of relevant articles for the rotation can be found on the GIM website along with a perioperative reading repository in the GIM OneDrive folder (a link to which has been sent to all GIM residents)听 The resident is expected and strongly encouraged to read and review these articles during the rotation.

The Perioperative Medicine Journal Club takes place once per month on Thursdays at 7:30 am. GIM residents are required to attend the Journal Club while on their Preop rotation and may be asked to present.

Attendance in longitudinal GIM clinic in weeks 1 and 3 alternating with preoperative medicine outpatient clinic in weeks 2 and 4 is mandatory. GIM residents are strongly encouraged not to book vacation or study leave during this rotation so as to maximize exposure to preoperative medicine.

Community GIM

The resident is required to complete at least 4 weeks in a community centre with few non-GIM medical subspecialists in the CoD stage of training. The resident will be asked听four months before the start of the PGY4 year to select three choices for their听community GIM听sites and the periods during which the resident would want to go. The CHIME Internal Medicine听coordinating office (which is administered by Ms. Maureen Dowd) will take these into consideration when finalizing the CHIME schedule for all levels of internal medicine residents (PGY1-5). Any requests for schedule changes must first be communicated to the GIM Program administrator who will determine if they get forwarded to the CHIME Internal Medicine coordinating office for consideration. Once the community GIM听site rotations have been scheduled, the resident will receive instructions from the Distributed Medical Education Office (DME)听in conjunction with the Program Office on how to arrange for lodging and travel, which will involve communicating with the CHIME site specific coordinators and administrators. Residents are expected to take call duties while on their community GIM required training experience. It is the resident芒鈧劉s responsibility to contact the CHIME site administrators at least 3 weeks ahead of time to submit their requests and get the call schedules. Global Health Initiative

成人VR视频 Global Health elective to Haiti for the core IM residents was expanded to include GIM residents.

Mike Quon was our first resident to participate and provided clinical teaching and care alongside staff Internists from 成人VR视频 and Haiti for 成人VR视频 core IM residents and Haitian Family Medicine residents. The experience was extremely rewarding, both personally and professionally, for all of the participants.

The elective is open to R5s and can be counted as one of the required Jr Attending periods. Anyone interested should contact Audrey Weizmann at aweizman [at] jgh.mcgill.ca.听

Obstetrical Medicine at MUHC

Eight weeks of听obstetric medicine听must be completed in the CoD stage of training, with at least 4 weeks听done outside 成人VR视频.听 Please contact Audrey Weizmann (aweizman [at] jgh.mcgill.ca, 514-340-8222, ext. 23942) to arrange a rotation at the MUHC (MGH and RVH-Glen). Another 4 weeks can be done at non-成人VR视频 sites such as Ste Justine and the CHUM. The Program administrator will book this required training experience as she becomes advised of the number of spots available for training of 成人VR视频 GIM residents. The sites outside Quebec and the CMQ need advance notice so please inform the Program admin at least 4 months ahead of time.

Clinical requirements for residents at the MUHC include attendance at ALL scheduled clinics described below, as well as round on the inpatient pregnancy consultation service at RVH and participate in Obstetrical Medicine/Maternal-Fetal Medicine rounds.

Teaching in addition to Clinical Case Review

A reading list will be provided at the start of the rotation. Residents are expected to read all 芒鈧搃mportant芒鈧 obstetrical medicine topics and encouraged to read other articles.

There is a Medical Obstetrics Journal Club held every 3rd Friday of the academic period at the RVH.听 The content and topic of choice is to be decided by the Internal Medicine Attending and fellow at the beginning of the rotation.

There will also be an Obstetric Medicine/Maternal-Fetal Medicine Academic session where GIM residents may be asked to present clinical cases.听 These will occur the 4th Friday of each period.听 Attendance and participation is mandatory.

Interactive online (CANCOM) cases are encouraged and may be reviewed/discussed with staff (). You can also find dates of upcoming GEMOQ meetings and past web conferences on this site.

In-Patient Consulting Service

You will be first call for obstetrical medicine consultations and follow ups weekdays Monday-Friday. You should liaise with the Obstetrics/Gynecology resident early in the rotation and regularly (each morning) to discuss cases.

Joint in-patient Obstetrics/MFM/Obs Med rounds will occur Friday mornings at 7:30am.

Please add new patients/update the Obs Med in-patient list on Oacis (can be found by searching lists that were created by Leora Birnbaum). Please do not delete any names from the list as it is also used for research purposes.听 Your name must be added as a list user in order for you to add names to the Oacis list - this should be done before the start of your rotation. Please contact Dr Natalie Dayan if you find your list privileges inaccurate.

Administrative Notes

One or both Obstetric Medicine RTEs can be done outside 成人VR视频 at any hospital with an established Obstetrics Medicine Program.听 The program administrator will book this rotation as she becomes advised of the number of periods for the training of 成人VR视频 GIM Residents.

The sites outside Quebec and the CMQ require advance notice.听 Audrey will need at least 4 months notice.

Attendance at GIM Clinic at the JGH and MUHC will continue听in weeks 1 and 3 only and clinic day can be changed if it听conflicts with a scheduled Med Obs clinic.听 It is the resident芒鈧劉s responsibility to contact the GIM Clinic director and administrator about such conflicts.

Please remember that attendance at all academic activities (Obstetrical Medicine and GIM program) is mandatory during this rotation.
