The specimen shows fibrous thickening and fusion of two aortic valve leaflets; a median raphe suggestive of a congenitally bicuspid valve is evident behind (arrow). The third leaflet is unremarkable. There is no evidence of acute endocarditis.

Comment

No clinical information is available. The descriptive card refers to "fibrinous tags" on the fused leaflets; these are likely fibroelastic extensions of the endocardium unrelated to infection. The perforation referred to on the card and indicated by an arrow in Rodin鈥檚 illustration (B) is in fact a depression at the base of the two fused leaflets; there is no evidence of acute endocarditis or perforation. It is possible that this is really specimen No 21 (the descriptive card having been mistakenly switched with the one now on that specimen). One argument in favour of this hypothesis is the reference听on the descriptive card of No 21; the protocol findings resemble the abnormalities in this specimen. However, the terminology in the protocol is confusing and it is difficult to be certain.
