Characteristics of individuals eligible for the clinic:
Counselling is intended for individuals who are interested in having an estimate of their personal risk for developing cancer. Those at a higher than average risk of developing certain cancers can benefit from new and current screening tests and prevention methods, whereas those at lower risk can be reassured that their risk for developing cancer is not as high as they thought.

The steps to take:
The first step is to obtain a referral or consultation from your Doctor. For the referral, please make sure of the following:
- Please provide your most up-to-date contact information; that is, your mailing address, your phone number (and fax number, if you have one), and if you already have a file at the Jewish General Hospital, your hospital card number
- Please clearly indicate your Doctor鈥檚 first and last name
- If you have had cancer and were operated on at a hospital other than the Jewish General Hospital, please provide a copy of the pathology report (basically a written record) of that surgery in addition to the referral. If there was no related surgery in the past, then the referral alone is enough.
The second and last step is to send the documents by fax to the secretary at (514) 340-8712.
Timeline:
Once your referral has been received, it will be reviewed by the Genetics team and a questionnaire asking details about your family history will be mailed to the address you have provided. You should fill this questionnaire as completely as possible and return it in the self-addressed envelope enclosed to the Department of Medical Genetics.
Please understand that in most circumstances an appointment can only be given once the Genetics team has received your questionnaire.
Once the questionnaire has been received, an appointment will be booked based on urgency. The waiting period may range from 3 months to 6 months depending on your circumstances.
